What the Program Provides

- Stays range from approximately two weeks to 24 months, depending on individual need and progress toward permanent housing.

- The program offers both limited-time transitional placements and connections to longer-term or permanent housing resources.

- Each participant receives case management and care management support throughout their transitional housing stay.

- We coordinate treatment and placement for individuals managing substance use disorder or severe medical, mental, or behavioral health conditions.
