How We May Use and Disclose Your Medical Information

BLH may use and share your protected health information for treatment, payment, and health care operations, and as otherwise required or permitted by law, without your specific permission. Any use or disclosure that falls outside those purposes will be made only with your written authorization, which you may revoke at any time.

Uses and Disclosures for Treatment, Payment, and Health Care Operations

We may share your information with the providers and partners involved in your care, and with the insurers and programs that pay for your services. We may also use it for internal operations such as quality review, care coordination, fundraising contact (which you may opt out of), and required reporting — always limited to what is appropriate for the purpose.

Your Rights Regarding Your Medical Information

You have the right to:

Inspect and request a copy of your records.
Request a correction to information you believe is incomplete or inaccurate.
Receive an accounting of certain disclosures of your information.
Request reasonable restrictions on how your information is used or shared.
Request that we communicate with you confidentially or by alternative means.

Breach Notification

In the event of a breach of unsecured protected health information, BLH will notify the affected individuals within 60 days of discovering the breach.

Changes to This Notice

BLH reserves the right to revise this notice at any time. Any revised notice will apply to all protected health information we maintain, both current and future.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with BLH or with the Secretary of the U.S. Department of Health and Human Services. You will not be penalized or retaliated against for filing a complaint.

Any use or disclosure of your medical information not described in this notice will be made only with your written authorization.
