BLH's Population Health Initiative works at the system level — partnering with managed care organizations (MCOs), hospitals, and physician practices to identify at-risk members and deliver coordinated, evidence-based interventions before conditions escalate.

By addressing social determinants of health alongside clinical needs, our approach reduces unnecessary emergency room visits, observation stays, and inpatient admissions — improving outcomes across entire populations.
