In partnership with the Maryland Developmental Disabilities Administration, BLH provides Coordination of Community Services to help individuals with intellectual and developmental disabilities live full, self-directed lives.

BLH's DDA program provides Coordination of Community Services (CCS) — a structured, person-centered approach to planning that supports individuals with intellectual and developmental disabilities through every stage of life.

From eligibility assessment and person-centered planning to waiver enrollment support and ongoing service coordination, our team guides participants and families through the full DDA process.

How It Works

The CCS Process

Eligibility Assessment
We work with the DDA to conduct eligibility assessments that determine the appropriate level of support and services for each individual.
Person-Centered Planning
Our coordinators facilitate planning focused on meaningful life goals — employment, community participation, independent living, and more — determined by the individual and their support network.
Waiver Waiting List Support
For individuals on a DDA waiver waiting list, we provide coordination and connection to available services while enrollment is pending.
Youth Transition Assistance
We provide dedicated transition support for youth moving from school-based services to adult programs, ensuring continuity of care and clear next steps.
Transfer Timeline
Individuals transferring to BLH as their CCS provider should expect the process to take approximately 30 to 60 days to complete.
