No single organization can address every need on its own. That is why BLH has spent more than two decades building relationships across the full spectrum of care — from families and individuals to hospitals, government agencies, and the community organizations working on the ground every day.

Who We Partner With

Our partnerships span five types of organizations and stakeholders:

Individuals & Families
The people we serve and the family members and caregivers who support them are our most important partners. Care works best when everyone is included in the planning.
Healthcare Systems
Hospitals, physicians, and managed care organizations partner with BLH to ensure smooth care transitions, reduce readmissions, and coordinate services across the continuum of care.
Long-term Services & Supports (Medicaid & Medicare)
We work closely with Medicaid and Medicare programs to connect eligible individuals with the long-term support and services that help them live safely and independently.
Government & Policy Makers
BLH participates in national, regional, and local initiatives addressing disabilities, care transitions, and affordable housing — advocating for the policy changes that most benefit the people we serve.
Community Organizations
Local organizations and community providers extend the reach of our work, filling gaps and connecting individuals with the trusted resources already present in their neighborhoods.
