Who We Serve
Most people will rather live at home independently than live in an institution. Our agency has made it our goal to make this dream possible for our participants. We have selected a group of experts that will assist and support our participants in leaving a nursing home and moving to a home in the community. This team is also skilled in supporting our participants, ensuring that they receive the care they need to remain in their homes. Our housing specialists have also assisted participants participating in the older adult wavier program when they needed to move to a more independent living, to accommodate the special needs of their family member.
